As the covered party, it is your responsibility to keep up to date with your plan coverage details. We make ever effort to verify insurance coverage prior to your appointment; validity is subject to the accuracy of information provided by your insurance company. In order to streamline confirmation efforts of benefits and coverage, please provide insurance information 3 working days before your appointment.

Please provide both your medical and vision insurance information. As a medical provider, we often file insurance for medical treatments as specific diagnoses are medical in nature. In addition, some medical plans include coverage for annual vision exams. We routinely coordinate benefits between your medical insurance and your vision insurance plan.)
